Certificate of Registered Notification 

  

The certificate of registered notification is the document which certifies you have notified a municipal office of births, deaths, marriages or divorces.
We have two types of certificate ; Certificate of Acceptance of Notification (Todokede Juri Shomeisho) and Copy of Notification Document (Todokede-sho no Utsushi)
When you need these certificates, you are required to fill out an application form for a certificate of registered notification at the municipal office where you have made the notification.
If you have notified Kakogawa City Office, you can make applications at the Citizens' Affairs Division in Kakogawa City Office or at one of 10 branch offices.

When you need Certificate of Registered Notification...

Applicant

Those who have made the notification can make applications in person or through a proxy.

Items for Application

In the case of an application through a proxy, a Letter of Attorney. (not required for your family members.)

Certificate Fee

Both are 350 yen per copy
